Nursing Care After Hospital Discharge

The first weeks at home are the riskiest. Our nurses help you recover safely after leaving Westmead, Blacktown, Auburn or other Western Sydney hospitals.

Good Nurse is based in Westmead, minutes from Westmead Hospital and Westmead Private Hospital. We help people settle home safely after a hospital stay, with Registered Nurses for clinical care and support workers for everyday help while you recover.

What we help with

Wound and dressing careFollowing the hospital's dressing plan and watching for infection.
Medication checksMaking sure new medicines are understood and taken safely.
ObservationsBlood pressure, glucose, temperature and other checks your doctor asks for.
Personal careSafe showering, dressing and toileting while you regain strength.
Falls preventionA home safety check and practical changes to reduce falls.
Help at homeMeals, shopping, cleaning and transport to follow-up appointments.

How it works

  1. Ideally we speak with you or your family before discharge to plan the first visit.
  2. An RN visits early, often the day after discharge, and writes your care plan.
  3. Care starts higher and steps down as you recover.
  4. We keep your GP informed and escalate early if anything changes.

Funding

Older people can use Support at Home, including the Restorative Care Pathway for short-term recovery. NDIS participants can use their plan. Private short-term care is also available. Not sure what your plan covers? Call us and we'll go through it with you.

Frequently asked questions

Can you start the day after I leave hospital?

Often, yes. The earlier you call before discharge, the easier it is to arrange.

Which hospitals do you cover?

We support people going home from Westmead Hospital, Westmead Private, Blacktown, Auburn, Mount Druitt, Norwest Private, Hills Private and other Sydney hospitals, as long as home is in our service area.

Is there short-term funding for recovery?

For older people, the Support at Home Restorative Care Pathway can fund short-term care. Ask the hospital team or My Aged Care about an assessment.

Can family organise this for me?

Yes. Family, hospital staff and GPs can all contact us or use our referral form.
