Palliative Care at Home in Western Sydney
Gentle, nurse-led care that helps people stay at home with the people they love, with support for the whole family.
Many people want to spend their last months at home. Good Nurse works alongside your GP and specialist palliative care team to make that possible, with Registered Nurses for clinical care and familiar support workers for personal care, overnight support and respite.
What we help with
How it works
- A nurse meets you and your family to understand wishes, needs and the advance care plan.
- We coordinate with your GP and specialist palliative care service.
- A small, consistent team provides care, so you're not meeting strangers.
- Care adjusts quickly as needs change.
Funding
For older people, the Support at Home End-of-Life Pathway can provide extra funding (around $25,000 over 12 weeks) for people with a prognosis of 3 months or less. NDIS participants may use their plan. Private care is also available. Not sure what your plan covers? Call us and we'll go through it with you.
